I hereby give Seward Memorial Library the absolute right and permission to use a photograph in which I am pictured (or in which a minor who I am parent or guardian to is pictured):
1. In promotional publications, including, but not limited to: catalogs, e-newsletters, flyers, brochures, posters, video public service announcements or other electronic and print publications.
2. To use my (or my child's) name, if applicable.
3. To use, reuse, publish and republish the same in any medium for any purpose. This permission includes mass media.
4. To use the name of the city or place where the photograph was taken, if Seward Memorial Library so chooses.
I hereby waive any right that I may have to inspect or approve the final photograph or the use to which it may be applied and release Seward Memorial Library from any and all claims, including claims for libel, arising out of the use of the organizational profile or photograph.
